You are able to do this by either:

• Using the online contact detail link and complete the sections which are applicable.
• Come into the surgery and complete a short form at reception and we will amend your details. The forms can be found in the entrance foyer with the repeat prescription box.
• Telephone the surgery and our reception staff will be happy to amend your details.
