If you are to be referred by your GP you will be given a booklet on Choosing your Hospital and also a letter describing what happens once your referral is typed by the secretaries.

If you have been referred to a specialist in column A:-

You will be sent a unique number and a password to enable you to arrange your appointment. Please do not lose this information; you will need it if you have to contact the centre on any future occasion.

Please contact the “Choose & Book Centre” following the instructions on the sheets that you will be sent. This will enable you to arrange an appointment suitable to you.

If you have been referred to a specialist in Column B:-

You will be contacted by one of the secretarial department and given the choice of appointments and hospitals. If by any chance we cannot contact or you or you are out at work all day please contact us on 0844 815 1571 between the hours of 9am and 5pm and we will arrange the appointment this way.
